Fun Run Liability Release Waiver Form

Every rider age 18 or older must sign a form  You may sign for your minor children on the same form.

I certify that I (and any minor children under my care, custody or control) participating in this fun (poker) run here by release, remise and forever discharge the Blythe Chamber of Commerce and staff from any and all liability, damages, cause of action, or other civil remedy arising from any injury, loss or damage or death that occurs by my own cause or the cause of others, because of my participation in or at the event on property in use by the Blythe Chamber of Commerce. 

That I am participating in this even on a voluntary basis, and having considered the danger associated with the use of an atv, utv, jeep, motorcycle or other mode of transportation, and do hereby acknowledge that I am joining in with my own risk as well as riders, I am fully aware of the dangers associated herein. I agree not to pursue any action, or cause any action of any nature against the organization, or property owners, for any loss sustained because of my attendance at and or participation in the event. 

By participating, I agree that no person shall, while on trail,

1. Be impaired by drugs or alcohol

2. Drive without a helmet or safety belts if applicable.

3. Destroy trail or private property

4. Dump trash or litter
